This Service Agreement Template is for the delivery of Commonwealth Home Support (CSHP) Services under the Support at Home Program. It includes Terms and Conditions relevant to, and necessary for, a binding agreement to deliver aged care services in the home to those receiving CHSP funding, but which must now be deliered line with the new Aged Care Act 2024. This template considers those delivering lower risk services. For providers of services in higher risk categories, we recommend you consider the full Support at Home Service Agreement.
